Philip Chism, the 15-year-old charged with raping and killing his Danvers High School math teacher, showed signs of psychosis after allegedly attacking a clinician at a Dorchester juvenile detention facility.

The Boston Globe reports that the teen, who is being held in the murder of Colleen Ritzer, was armed with a pencil when he followed a counselor into a staff bathroom and choked, stabbed, and punched her.

After being restrained, Chism “presented immediately with psychotic symptoms, yelling, screaming incoherently, [and] foaming at the mouth,” the Globe reported, citing a Tuesday court filing.

The incident is eerily similar to the October slaying of Ritzer. Prosecutors say Chism attacked the 24-year-old teacher in a high school restroom, then dragged her body to a wooded area behind the school.

He is being held without bail after pleading not guilty.
