A woman was killed when an airborne manhole cover slammed into her car on Interstate 93 in Boston on Friday morning, WCVB reported.

The driver was headed south just before 8 a.m., approaching the O’Neill Tunnel exit.

“Preliminary investigation suggests it struck the victim’s windshield somewhere in that vicinity. After impact, the vehicle continued southbound in the left lane of Route 93 reaching near the area of East Berkeley Street before hitting the wall on the left shoulder and coming to a stop,” State Police spokesman Dave Procopio said in a statement referenced by CBS.

Details about the victim’s identity have not been released, according to WCVB.

“What caused the manhole to become dislodged is part of the ongoing investigation,” Procopio’s statement was quoted by CBS.
